Inpatient Rehabilitation Service
MD was transferred to Bridge House Rehabilitation center, from a similar setting offering rehabilitation care. MD had been discharged there from hospital with the aim of improving her mobility in order to return home, however, instead she had spent the entire 10 months in bed, completely immobile despite physio input.
Initial assessment
The client was hoist transfer, but generally cared for in bed 24/7. She was highly anxious of falling. She was cognitively well and determined to improve her physical state.
Rehabilitation
MD She had become independent in her mobility with walking frame at our rehabilitation centre and returned home with 3 times daily home care . She has been receiving home visit physiotherapy and reablement . Now she is independent in making hot drinks and meals and has reduced her care to twice daily . She has also started practicing shower transfers with the aim of using shower in coming weeks.
