Care Home Rehabilitation: Our New Partnership with Winchcombe Place
We are pleased to announce a new partnership between Therapies on Thames and Winchcombe Place Care Home in Newbury.
Through this partnership, our physiotherapy and occupational therapy specialists will work directly with residents and the Winchcombe Place care team to support mobility, independence and day-to-day wellbeing.
Rather than providing therapy as a standalone service, the partnership brings rehabilitation expertise into the home on an ongoing basis. This allows our therapists and the care team to work closely together, monitor residents’ changing needs and build practical rehabilitation support into everyday care.

Supporting Residents from the Moment They Arrive
Every new resident at Winchcombe Place will receive a complimentary initial assessment from Therapies on Thames.
The assessment will help us develop a clear understanding of the resident’s mobility, independence and current support needs. It also provides an opportunity to identify any concerns relating to balance, transfers, walking, falls risk or everyday activities at an early stage.
This information can then be shared with the wider care team, helping everyone involved understand how the resident can be supported safely and confidently from the beginning of their time at the home.
For some residents, the focus may be on maintaining their existing mobility and independence. Others may benefit from rehabilitation following an illness, hospital admission or change in their physical abilities.
Whatever the circumstances, the starting point will always be the individual resident, their needs and the activities that matter to them.
Ongoing Physiotherapy and Occupational Therapy Support
The partnership extends well beyond the initial assessment.
Therapies on Thames will provide ongoing therapy oversight, helping the Winchcombe Place team review residents’ mobility, independence and rehabilitation needs as these develop.
Physiotherapy support may include assessing walking, strength, balance, transfers and physical confidence. Our physiotherapists can also recommend suitable exercises and practical strategies to help residents maintain movement or rebuild their abilities following a period of reduced activity.
Occupational therapy focuses on the everyday tasks that help people remain involved in their own lives. This may include getting dressed, moving around their room, using the bathroom, sitting comfortably or participating in meaningful activities.
Our occupational therapists consider both the resident’s abilities and their surroundings, identifying adjustments, equipment or alternative techniques that may make daily tasks safer and more manageable.
By providing both physiotherapy and occupational therapy, we can look at mobility and independence as connected parts of a resident’s overall wellbeing.
Regular Falls and Mobility Assessments
Falls prevention is an important part of supporting older people, but it is rarely as simple as addressing one isolated risk.
A person’s likelihood of falling may be affected by their strength, balance, mobility, medication, vision, environment or confidence. Their needs can also change following an illness or a period of reduced activity.
As part of the partnership, Therapies on Thames will provide regular falls and mobility assessments for residents at Winchcombe Place.
These assessments can help identify emerging concerns, review whether existing support remains appropriate and guide practical measures to reduce avoidable risks.
Where a fall has already occurred, our therapists can look at the possible contributing factors and recommend appropriate next steps. The aim is not simply to restrict movement, but to help each resident remain as active and independent as possible while receiving the right level of support.
Exercise and Information Sessions for Residents
Residents will also have access to group exercise and information sessions delivered by Therapies on Thames.
Group sessions can provide an enjoyable and social way for people to work on movement, strength and confidence. Exercises can be adapted to accommodate different abilities, including residents who take part from a seated position.
Alongside the physical benefits, shared sessions can encourage participation and help make movement a more familiar part of life within the home.
Information sessions will also give residents opportunities to learn more about subjects connected to mobility, falls prevention, independence and healthy movement.
These sessions are intended to be practical and accessible, helping residents understand how small, manageable actions can support their wellbeing.
Sharing Expertise with the Care Team
A key part of this new partnership is the support provided to the Winchcombe Place care team.
Therapies on Thames will deliver specialist training to help colleagues build their understanding of mobility, rehabilitation and safe day-to-day support.
This means the knowledge gained through a therapy assessment does not remain with the visiting clinician. It becomes part of the way the wider team supports that resident throughout the week.
Training may cover areas such as mobility assistance, transfers, falls prevention, positioning and supporting residents with recommended exercises or rehabilitation plans.
Our therapists will also remain available to provide guidance as residents’ needs change, creating a more joined-up approach between clinical therapy and everyday care.
